Health Care Services For the Homeless

Having access to health care services when homeless can often be difficult.

The Health Department collaborates with many other health providers in Lexington to help our homeless community get health care when they are sick as well as care to stay healthy.

A nurse travels throughout Fayette County to find homeless persons and help them to get health and social services they may need.

Our Primary Care Center can provide

  • clinical health care services such as diagnosis and treatment of medical conditions, lab tests, x-rays, and basic medications
  • social services/case management to assist homeless persons in accessing entitlement programs and in meeting other basic human needs
  • referrals for substance abuse and mental health services, in collaboration with the Comprehensive Care Center.

This federally funded grant program is a collaborative effort coordinated by the Health Department's Primary Care Center in conjunction with the Hope Center, the UK College of Nursing, Comprehensive Care Center and the Health Department's Community Nursing Division.
