Primary Care Center

The Primary Care Center of the Lexington-Fayette County Health Department is located at 650 Newtown Pike . The Primary Care Center provides medical and dental clinic services for children and adults.

Pediatric Services

Pediatric services are available for children from birth to 20 years old. Services include sick-child care as well as preventive services, such as Well Child examinations, sports physicals, Pap smears, and family planning services.

Adult Care

Adult care is available for patients 21 years or older. Services include preventive care, such as physicals, pap smears, breast and cervical cancer screening, family planning services and care for acute and chronic illnesses.

Dental Services

Dental services are available for children and adults. Services include preventive and restorative dental care.

Pharmacy

An on-site pharmacy is available to fill new prescriptions as well as provide refills as prescribed.

Hours of Operation

Medical Clinic:

  • Monday, Tuesday, Thursday 8 a.m. - 6:30 p.m.
  • Friday 8 a.m. - 4:30 p.m.
  • First and third Wednesday of each month 10 a.m - 4:30 p.m.
  • All other Wednesdays 8 a.m. - 6:30 p.m.

Dental Clinic:

  • Monday, Thursday, Friday 8 a.m. - 4:30 p.m.
  • Tuesday, Wednesday 8 a.m. - 6:30 p.m.

Pharmacy:

  • Monday, Tuesday, Thursday 9 a.m. - 6:30 p.m.
  • Friday 9 a.m. - 4:30 p.m.
  • First and third Wednesday of each month 10 a.m - 4:30 p.m.
  • All other Wednesdays 9 a.m. - 6:30 p.m.

Charges

A minimum fee of $10.00 is required at the time of visit in most cases. Medicaid, Medicare and private insurances are accepted. Proof of income is required to qualify for a sliding scale of reduced fees based on household income. For questions about charges and billing call 288-2494.

For your appointment, bring the following:

  1. $10.00 for minimum fee charge.
  2. Private Insurance card, Medicaid or Medicare card for each visit.
  3. Children and adolescents who are less than 18 years old, must be accompanied by a parent or guardian. Guardians must present proof of guardianship for the initial visit.
  4. Proof of income to qualify for clinic and pharmacy sliding scale discounts.
  5. List of all medications (prescribed and over-the counter) you are presently taking.
  6. Records and other information from other doctors’ offices or emergency room visits.
  7. Immunization records.
